Second Duplicate Passport Instructions
| 1. SUBMIT EXPIRED PASSPORT |
• Your expired passport must have been issued less than 15 years ago |
2. COMPLETE THE APPLICATION FORM (DS-82) (click here) |
• Fill out the DS-82 (Passport Renewal Application form) IMPORTANT: YOUR SIGNATURE MUST APPEAR THE SAME ON ALL DOCUMENTS. DO NOT LET ANYONE ELSE EXCEPT THE APPLICANT SIGN THE FORM. PLEASE PRINT APPLICATION FORM ON SEPERATE PAGES NOT DOUBLE SIDED. IF YOUR SIGNATURE HAS CHANGED OVER THE YEARS PLEASE SUBMIT A COPY OF CURRENT SIGNATURE IDENTIFICATION. |
3. COMPLETE PASSPORT LETTER OF AUTHORIZATION (click here) |
| • Print two copies and complete the Passport Processing Authorization Letter. • Fill out this form by hand and sign the bottom using blue or black ink. IMPORTANT: YOUR SIGNATURE MUST APPEAR THE SAME ON ALL DOCUMENTS |
4. COMPLETE A REQUEST LETTER |
| • Fill out a request letter addressed to the U.S. Passport Services explaining exactly why you are requesting a second passport. Sign the letter the same way you signed the DS-82 and include your date of birth next to your signature. • You can use a blank sheet of paper for this. |
5. PASSPORT PHOTOGRAPHS |
| • Two identical passport photos are required for a new passport. • Photo requirements click here. |
6. PROOF OF INTERNATIONAL TRAVEL |
• Provide two copies of one of the following travel documents below proving your international travel within 14 days of this application submission: |
