Replacing a lost or stolen passport is essentially the same process as applying for a new passport. The biggest difference between replacing a lost passport and applying for a brand new one is that in order to replace a lost passport, you will have to sign a document that formally states that your passport has gone missing. This document, also known as the DS-64, needs to be filled out and signed by the passport applicant, and sent to us along with the DS-11, or Application for a New U.S. Passport. In addition to your DS-11 and DS-64, you will need to send in a number of identifying documents like your ORIGINAL Birth Certificate (if you were born in the U.S.), or your Naturalization Certificate.

Once you have gathered all of the necessary documents, you must go to your local Post Office or County Clerk’s office to sign the DS-11 in front of a government employee, who will seal these documents into an official envelope which you will then bring to your local FedEx office and send it to our office via Priority Overnight shipping.
