How To: Our Online Application Wizard
One of the most common questions we get here at Fastport Passport has to do with confusion about our online Application Wizard.
Once you have placed an order for a new, renewed, or replacement for a lost or stolen passport, you will receive a set of instructions in your email inbox on how to next proceed. The first step prompts you to fill out the application either by hand or with our Online Wizard.
Should you choose to fill out your application via our Online Wizard, the application contains boxes asking what kind of passport you want, as well as what type of service, and lists fees for these services. This is where people get confused. You can check the boxes for “Passport Book” and “Expedited Service”, and their fees; However, you have already by this point paid for our expedited service fee, so checking these boxes will not actually make you pay for any additional service or payment.
At the end of the online application, simply click “Create” and you will print out your application with your home computer. No extra fee will be charged to your card.

- DS-11, or Application for U.S. Passport
- DS-82, Application for Passport Renewals
- DS-64, Statement Regarding a Lost or Stolen Passport
- and the DS-3053, or the Statement of Consent to be filled out and notarized by an absentee parent
