New Passport Checklist Adults 16 years and older

Required Items:

1. Proof of US Citizenship (Birth Certificate, Certificate of Birth Abroad, Naturalization Papers)

2. Primary Identification (Drivers license valid for more than 6 months, if you do not have a drivers license you need as much additional identification as possible. Non drivers ID, social security card, health insurance card, credit card, library card, high school year book. IMPORTANT you must bring an identifying witness with you who has a valid drivers license or valid passport.

3. Passport Photos 2×2 on a white back round (Note: you cannot wear a white shirt in the photo must be a darker color for your passport photo, you can obtain passport photos for free in any of Fastport Passport Locations. You can also obtain passport photos at a local CVS, or Walgreens.
